This is strange, it seems map was "CTF-1HS-Bedrooms][a" although that maps contains not a single teleport, which was the one causing the glitch. I have no clue why the glitch happened there.
If it happens again in other map please report it so I can see what they have in common, and address the issue and say what is causing it to happen.

That's an extreme stretch to be the reason behind it. Maybe there's something in depth there that is causing the issue. I'll take a detailed look when I get the chance, not a single brush or actor will go unnoticed.
